Review of Diagnosis and Management of Resistant Hypertension

To the Editor We read with interest the recent comprehensive Review on the management of resistant hypertension. We have concerns, however, about a disconnect between the article’s evidence and the article’s Abstract conclusion, which seemed to equate the efficacy of spironolactone and renal denervation. Elevating renal denervation to the same level as spironolactone is not endorsed by the 2025 American Heart Association/American College of Cardiology high blood pressure guidelines. In fact, these guidelines discuss renal denervation as a potential consideration only after mineralocorticoid receptor antagonists.
