Three Pillars of Effective Scientific Communication

Physicians, clinician-scientists, and researchers communicate their findings through publications, grant applications, hospital grand rounds, and oral and poster presentations at regional, national, and international meetings. In 2025, JAMA Otolaryngology–Head & Neck Surgery received 1730 manuscripts for consideration for publication; the US National Institutes of Health received 62 592 applications for research project grants, 6065 of which were from early-stage investigators; and there are currently 1 085 661 citations in PubMed with a publication date of 2025. Yet, despite this extraordinary volume, failure to communicate efficiently and effectively has increasingly become a hallmark of medical research. Michael Crichton—a physician before he became a novelist and the creator of Jurassic Park and the television medical drama, E.R.—captured this paradox in a 1975 essay in the New England Journal of Medicine titled “Sounding Board: Medical Obfuscation: Structure and Function.” Its central argument, made with the precision of a clinician and the wit of a novelist, was that most medical writing is not merely unclear—it is functionally unclear: a calculated attempt to confuse the reader, and he identified 10 recurring faults.
